Social prescribing is an NHS service that enables health professionals to discuss and refer people to a range of local community services include art groups, gardening, befriending, cookery, healthy eating advice and a range of sports to support improvements in health and wellbeing.

The referrals generally, but not exclusively, come from professionals working in primary care settings, for example, GPs or practice nurses with the aim to address people’s needs in a holistic way. By sharing your views and experiences, we can initiate improvements and reduce barriers to access so more people can be supported to take control of their health.

If you require the survey in paper form, an alternative language, BSL or another format please email sxicb.involvement@nhs.net . The survey will close on Friday 15 September 2023.
