Share your thoughts on hospital discharge
Have you or a loved one been discharged from hospital in the last 12 months? If yes, we want to hear from you.
Healthwatch England would like you to provide details about your or your loved one's experience of being discharged from hospital, including to what extent you were involved in the planning for your discharge, the time you were discharged, whether your discharge was delayed and why and whether you got the support you needed after you were discharged from hospital.
They'll analyse the information you provide to feedback to the NHS how they they can improve the hospital discharge process for everyone. When they publish the results, they will not use any information that will identify you.
