Call for stories on hospital discharge

Some of you may have seen the Healthwatch England report called “Safely Home”
(http://bit.ly/1RLYP4H) today. It looks at the experience of people being discharged from hospital.

Healthwatch Brighton and Hove was one of 101 Healthwatch organisations around the country contributing to evidence. See our report here http://bit.ly/1KylGJ9. We found that patients felt they either left hospital too early with no warning or communication with their family/carers and/or they experienced long delays due to problems such as getting medication or social care arrangements in place at home.
